Child Case Manager

Full Time – Salary starts at $43,888 + unlimited monthly incentives

Taylor Life Center is looking for a dedicated Child Case Manager to provide person-centered case management from intake through discharge or transition. You’ll support kids and families dealing with mental illness, intellectual disabilities, or substance use challenges—helping them get the care and stability they deserve. Your role includes coordinating services, keeping clear documentation, and working with a team to ensure timely and compassionate support.


What you’ll do:

  • Manage a caseload with a minimum of 300 billable service units monthly (full-time)

  • Provide about 80% of services in home or community settings, aiming for at least one in-home visit monthly

  • Lead treatment plan meetings and coordinate with therapists, psychiatrists, PCPs, and other team members

  • Assess needs and match people with the right treatment interventions and community resources

  • Complete documentation (treatment plans, progress notes, assessments) within 24 hours of contact

  • Participate in quality improvement and staff meetings

  • Train and support colleagues to improve service delivery


What you need:

  • Qualified Certified Mental Health Professional (CMHP)

  • 3 years experience with a bachelor’s degree, or 1 year with a master’s degree, in human services (like Social Work, Psychology, Sociology, Criminal Justice)

  • Licensed in Michigan as LMSW, LLMSW, LMFT, LBSW, LPC, RN, or a related field

  • Valid Michigan driver’s license, auto insurance, and reliable vehicle use during work hours

Work conditions:

  • Physical: Standing, walking, lifting, kneeling, driving, physical intervention when needed

  • Mental: Reading, writing, decision making under pressure, confidentiality, leading others

  • Environmental: Working in community settings and client homes, possible exposure to aggressive behavior or poor hygiene, availability for some evenings and weekends
